Club Forms to Complete
In addition to registering each school year to ensure we have the most accurate information on your child(ren), there are a number of forms you may either be required to complete (such as medical), or may come to need to complete during the year. The forms linked below will enable you to do this.

Each time you register or re-register your child, you will need to complete the medical information form in addition to the annual registration form. This is because it carries important permissions and information that might change year to year. So it's crucial for your child's wellbeing that we have this as up to date as possible.

If you wish to register for ad hoc use of the club (very occasional use, not a semi-frequent pattern), please select this option on the registration form, and once registered and the registration fee paid, you can contact the club manager Nikki directly to see if space is available on the day you require. You can reach her best via email at nikki.m.stan@gmail.com if you are unable to speak with her in person at the club (she is not on the "club floor" every day)

If you need to change your sessions (for example, drop a breakfast club session, add an after school session), please complete the session change form. We will then update our termly register to reflect this, and let you know (in the case of additions) if it is not possible to accommodate your request.
